An I-9 Form (Employment Eligibility Verification) is required when companies hires new employees to verify their identity and employment authorization. All US employers must complete this form.

There are two pages:

  • Page 1: Section 1. Employee Information and Attestation and Preparer and/or Translator Certification
  • Page 2: Section 2. Employer or Authorized Representative Review and Verification and Section 3. Reverification and Rehires (To be completed and signed by employer or authorized representative.)
